Training Waiver and Release of Liability

In consideration of being allowed to participate in any hockey training sessions, camps, or events hosted by Pavlas Hockey LLC during the year 2025, the undersigned agrees to the following:

1. Assumption of Risk: I understand and acknowledge that ice hockey is a high-impact sport that involves physical contact, pucks, sticks, sharp skate blades, and movement on ice, all of which carry a risk of injury. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of participation.

2. Release of Liability: I hereby release and discharge Pavlas Hockey LLC, its owners, coaches, staff, volunteers, and any associated entities, including but not limited to the facility, rink management, and property owners, from any and all liability, claims, or demands arising from injury, illness, or damages sustained during participation in training activities.

3. Medical Consent: In case of injury or emergency, I authorize Pavlas Hockey LLC representatives to seek medical treatment for the participant. I understand that I am solely responsible for any resulting medical expenses.

4. Equipment and Conduct: Participants are responsible for bringing and wearing appropriate, certified hockey equipment at all times during training. Unsafe or unsportsmanlike conduct may result in removal from a session with no refund.

5. Photo/Video Release: I give permission for photos or videos taken during training to be used by Pavlas Hockey LLC for promotional or educational purposes.

6. Health and Safety: I confirm that the participant is in good health and has no medical condition that would prevent safe participation. I agree not to attend or send my child to training if experiencing symptoms of illness or contagious conditions.
