Pavlas Hockey LLC ("Company"), a South Carolina limited liability company.
Activity: Any and all hockey training sessions, camps, clinics, events, on-ice or off-ice activities, tryouts, or related programs hosted or operated by Pavlas Hockey LLC during the 2025-2026 season or any extensions (the "Activities").
I, the undersigned (participant if 18+, or parent/legal guardian if participant is a minor), in consideration of being permitted to participate in the Activities, agree as follows:
1. ASSUMPTION OF RISK I understand and acknowledge that ice hockey and related training activities are inherently dangerous sports involving high-speed movement, physical contact, pucks traveling at high speeds, sharp skates and sticks, falls on ice, collisions with other participants or objects, equipment use, and other hazards. These risks can lead to serious injuries such as concussions, fractures, lacerations, sprains, strains, spinal or head injuries, permanent disability, death, or property damage—whether arising from the nature of the sport, actions of other participants, ice or facility conditions, equipment, or any other cause.
I VOLUNTARILY AND KNOWINGLY ASSUME ALL SUCH RISKS, both known and unknown, and accept full personal responsibility for any harm, loss, or injury that may result from my/our participation.
2. RELEASE AND WAIVER OF LIABILITY In consideration of being permitted to participate in the Activities, I HEREBY RELEASE, WAIVE, DISCHARGE, AND AGREE NOT TO SUE Pavlas Hockey LLC, its owners, officers, directors, employees, coaches, instructors, volunteers, agents, contractors, insurers, affiliates, the rink/facility owners/operators, and all related entities/personnel (collectively, "Releasees") from ANY AND ALL LIABILITY, CLAIMS, DEMANDS, ACTIONS, OR CAUSES OF ACTION of any kind whatsoever for any loss, damage, expense, injury (including death or disability), illness, or property damage arising out of or related in any way to participation in the Activities or travel to/from them.
This release applies regardless of the cause, including but not limited to the inherent risks of the sport or any acts or omissions by the Releasees.
This release covers claims that might otherwise arise under ordinary circumstances but does not apply to claims arising from gross negligence, recklessness, willful or wanton conduct, or intentional acts (to the extent such claims cannot be waived under applicable law).
3. INDEMNITY AND HOLD HARMLESS I agree to indemnify, defend, and hold harmless the Releasees from any claims, losses, liabilities, damages, costs, expenses (including reasonable attorneys' fees), or demands brought by me, the participant, my/our heirs, or any third party arising from or related to my/participant's involvement in the Activities.
4. MEDICAL CONSENT AND RESPONSIBILITY I certify that the participant is physically fit for hockey training with no known conditions preventing safe participation. In an emergency, I authorize Releasees to obtain medical treatment and agree to pay all related costs/expenses. I am solely responsible for medical insurance and expenses.5. EQUIPMENT AND CONDUCT Participants must wear properly fitted, certified hockey equipment (helmet with full cage/mask, etc.) meeting USA Hockey or applicable standards at all times. I agree to follow all rules; unsafe/unsportsmanlike conduct may result in removal without refund.
6. HEALTH AND SAFETY I confirm the participant has no symptoms of illness/contagious conditions and will not participate if symptomatic. I agree to comply with all health/safety protocols.7. PHOTO/VIDEO RELEASE I grant Pavlas Hockey LLC permission to photograph/video the participant during Activities for promotional, educational, or marketing purposes (no compensation).
8. VOLUNTARY AGREEMENT I have read this entire document, understand it fully, sign voluntarily without duress, and had opportunity to consult counsel. If any provision is invalid, the remainder remains enforceable.
9. GOVERNING LAW This Agreement is governed by the laws of the State of North Carolina (where the Activities primarily occur), without regard to conflict of laws principles.
10. CONDUCT AND PROGRAM REMOVAL Pavlas Hockey LLC is committed to providing a safe, respectful, and positive environment for all participants, coaches, and staff. Participants (and parents/guardians) agree to follow all program rules, behave respectfully toward others, and refrain from disruptive, unsafe, disrespectful, harassing, bullying, or unsportsmanlike conduct at any time during the Activities.
The Company reserves the right, in its sole discretion, to remove or suspend any participant from a session, camp, event, or the entire program for violations of conduct rules, including but not limited to behavior that endangers others, disrespects coaches/staff/other participants, disrupts training, or fails to comply with instructions.
In such cases, no refund or credit will be provided for any fees paid, sessions missed, or remaining program time. This policy helps ensure the safety and enjoyment of all involved.
